Loaner request

1. Call the customer service for availability.
2. Send a email confirmation at the customer service with the form.
3. Sets and implant tub will be sent.

Note: the return label is included.

After the case:

1. Send the DGF to customer service e-mail address.
2. Collect the Po from the Hospital and send it to Customer service e-mail address.

Consignment Process

1. Send the DGF to Customer service e-mail address.
2. Collect the Po from the hospital and send it to Customer service e-mail address.
3. Renewal of the implants used will be sent shortly
